About insurance
If your insurance requires a referral from your primary care physician, you must obtain it before your visit. If a referral is required and you are unable to obtain it before your appointment, you will be asked to reschedule or to pay for your visit in full.
We do not file all types of insurance and some procedures, such as in vitro fertilization, are not covered by some insurance providers. If you have any questions about your insurance coverage, our Patient Accounts Department will be glad to help you. Call them at (502) 271-5996, 8:00AM - 4:30PM, Monday through Friday.

Infertility Program Patients
When you arrive for your appointment with the physician, you MUST provide written documentation from your health insurance carrier of infertility benefits and coverage.
- If you do not provide the written documentation of such coverage, you will be expected to pay for the service on the date of your appointment.
- If you do not have infertility coverage, you will be expected to pay for the service on the date of your appointment.
- If you are not prepared to pay, you will be asked to reschedule your appointment.
